There are some surgery procedures that can reduce the zygomatic arch and zygoma. Some surgeons still utilize Frontal End Zygoma and Zygomatic Arch Osteotomy or Frontal and Rear End Zygoma and Zygomaitc Arch Osteotomy. Background Story

This beautiful girl has a pair of big smart eyes. At the first glance people will notice her beautiful eyes instantly. She doesn't speak a lot but her eye can tell you what she want to say. The first time she saw Dr. Ma for consultation and the second time for doing the surgery. She didn't demand for many goals but make her face smaller and natural because she didn't like her wide face. From the pre surgery photo in the front view the mid face is wide with a bulging out zygoma and zygomtic arch causing facial indentations both above and blow the zygomatic arch. The whole face is in a diamond shape. Post operatively the side facial outline is smooth, no obvious bulging out or indentation. After surgery the temporal and cheek region is fuller. This fuller effect in this areas is not by any means of augmentation but by the reduced size of zygomatic bone and zygomatic arch. And this reduction made the lateral facial surface even. On the contrary if a temporal and cheek augmentation is employed to make the facial outline smoother then the whole face will be wider or even end up in a swelling look after surgery.

In the right and left 45 degree angled before surgery photos the under zygomatic arch indentation is obvious as shown by the white arrow. The area of the zygomatic process is big as shown by the red arrow on the left side of the face and the zygomatic curvature protrudes to the lateral as shown by the red arrow on the right side of the face. After surgery the zygoamtic process becomes smaller and the zygomatic arch is no longer protruding to the lateral. The overall look of the face is more round and less bony.

In the chin up position pre operative photo the area between the lateral canthouses and the ears is fuller and bulging out to the lateral compared to the post surgery photo. This is not because the soft tissue shrinkage in these areas but the structural change of the zygomatic bone and the zygomatic arch. 

No matter in which angle of the face as showing in this page's photos there is no clue of facial sagginess happened comparing them pre and post surgically. On the contrary if we exam the photos carefully we could tell that when the facial bone is reduced after surgery the facial soft tissue is tighter and more lifted giving a younger looking. This kind of change can also be noticed in other facial bone reduction comparison photos in this website indicating that a right facial bone reduction surgery do lift the face somehow and make the surgery receiver younger instead of causing any facial sagginess. Another most frequently asked quest in everyday consultation is that if the facial bone reduction surgery accelerating aging. In Dr. Ma's opinion this is just a rumor from guessing.
