Twisted Hump Nose Plastic Surgery

The hump nose is mainly caused by the overgrowth of the bony nasal septum. The oversized septum lifts the nasal dorsum forming an augmentation in the middle of the dorsum like the hump of a camel. So a nose in this shape is called as "Hump Nose". The covering nasal bone restrict the enlargement of the developing bony nasal septum in some extent. If the restriction is too tight or the growth development of the bony septum is too fast there is not enough space in the nasal cavity for the septum to spread. The septum is then forced to become curved and the curved bony septum in turn pushes the dorsum deviated to one side or another. On top of that the cartilage septum also curved under the same mechanism and it then pushes the tip of the nose to the side, thus the whole nose is twisted. Twisted nose is very preva lent among hump nose patients. Twisted hump nose is often seen in Caucasian population.

According to the mechanism of the formation of the twisted hump nose it is imaginable that the nasal disfigures of this disorder include the deviation of the bony nose, the wry nasal septum, twisted nasal tip and even asymmetric nostrils. So in order to correct the twisted hump nose the deviated bony nose needs to be straightened, the wry septum and crooked nasal tip, nasal alar and even columella also need to be modified. Only a combined plastic surgery could possibly give an ideal after surgery result. The beautiful girl in this page had a twisted hump nose. In the front view picture the nose seemed switched to the left. The hump doesn't show well in this view. However as a result of the nasal bridge deviation the right side of the nasal body is bigger than the left. The coexist problem of wide nasal bone make the nose looks heavy and flat without definition, more like a hill than a mountain. During the hump removal rhinoplasy the bony hump at the dorsum is excised leaving the nasal bone bridge open like a open roof, leaving the dorsum of the nose flat and hollow in bone. In order to prevent this open roof deformity after the removal of the hump nasal bone osteotomy is done to move the two sides of nasal bones to meet in the center. So the nasal body becomes narrower at the same time. In this girl's case the pictures show that the middle part of her nose is narrower, the nasal outline on both sides are more vivid and the nose has its waist after the surgery.

In the angled front view picture the hump of the nose is clearly shown. The middle part of the dorsum is augmented making the tip of the nose even lower than the nasal bridge. This arrangement give an unnatural look of the nose. During the surgery the hump is removed and the root of the nose is augmented with implant thus the outline of the dorsum of the nose is straighter and more natural, the nasal tip is more defined.

In chin up position the originally left side swing columella is switched back to up right position. The up right columella make the nostrils more symmetric and both sides of the nasal alars are more symmetric also. All these changes give a prettier nasal triangle in chip up position.

In chin down position the before surgery nasal tip points to the left. After surgery the tip points to the middle of the chin. In general the deviated nose is classified into straight line deviation and "C" or "L" shape deviation according to the start point of the deviation. For the straight line deviation the start point is high. For the "C" shape deviation the start point is low or the middle part of the nose is twisted to one side while the root and tip of the nose are in the right position. This girl's nasal deviation started high and straightly deviated to the left without any curvature. So hers is a straight line nasal deviation. This kind of twisting mostly is caused by the simultaneous bony and cartilage septum deviation to the same side. The nasal bone, bony septum and cartilage septum need to be handled in correction surgeries for this kind of deviation. While the "C" shape nasal twisting mostly caused by the cartilage septum abnormality and it often can be corrected with just cartilage septum manipulations.

The after surgery photos was taken 4 months post surgery. At that point of post surgery time the swelling caused by scaring of the surgical trauma still exist. However the shape of the nose is relatively stable.
